This is a WORD format template which, that once signed will allow you to receive files, documents, reports or other information on behalf of the signer. 

This is helpful since often individuals don’t have the time to obtain the records themselves or customers would rather let someone else interact with the recordholder on their behalf.

How do I use this template?

1. This release contains fields that should be overwritten. Those fields are marked with “<” and “>” symbols.
2. Fill out your customer’s name
3. Have your customer sign and date it.

Can I customize the template? 

Yes. This template is available in ‘word’ format so that you may customize it to your liking.

Use the ‘save as’ feature on MS Word so you can continue to use this template over and over without having to re-purchase this template and to avoid writing over your template.
