Affidavit (or Statement) of Personal Knowledge
Affidavit (or Statement) of Personal Knowledge
What’s included when you purchase?
This downloaded WORD format document contains an affidavit of witness letter template. The second version includes a Jurat if the witness is going to notarize the Affidavit.
· Statement of Personal Knowledge
· Affidavit of Personal Knowledge (Notarized)
The difference between the two is that one requires notarization and the other doesn’t.
When would I need an Affidavit (or Statement) of Personal Knowledge?
In many immigration processes it is helpful to provide an Affidavit or Statement of Personal Knowledge. Some examples are when the applicant is submitting evidence that they are a person of good moral character, such as in Immigration Court proceedings or in USCIS waiver applications.
See www.uscis.gov for detailed information.
Can I customize the template?
Yes. This template is available in ‘word’ format so that you may customize it to your liking.
Use the ‘save as’ feature on MS Word so you can continue to use this template over and over without having to re-purchase this template and to avoid writing over your template.
Does the letter need to be notarized?
All ‘affidavits’ must be sworn to be true under oath and, according to California law, this means you must have a notary public execute the affidavit.
If you would like to present the statement without notarization, use the ‘statement of personal knowledge’.
